Get a Library Card

Library cards are free to Wisconsin residents. To apply, bring in a photo ID and proof of your current address, or fill out the online form below. Parents must sign for children under the age of 16.

We are a fine-free library, although you are responsible for material checked out on your library card, including material checked out on it by others unless you report the loss of your card.

Virtual Library Card Application

This form is for people who do not have a card from the Monticello Public Library, or any other library in the South Central Library System (SCLS). It will create a temporary account, valid for online resources and LINKcat holds. To upgrade your account for full access, you must bring a photo ID to the library during open hours. We will respond to all requests within one business day.
